Good Cause Exception
- If you had a fingerprint clearance card denied or suspended by the Arizona Department of Public Safety (DPS), please use the Board's Application Portal.
- If not using the Portal, please use this downloadable application form.
- Make sure that the letter from DPS says that you are eligible to request a good cause exception.
- Although the Board requires two reference forms, some applicants may want to submit additional reference forms.
- You should use the Board's Application Portal if you received a letter from either the Arizona Department of Child Safety (DCS) or Arizona Department of Economic Security (DES) saying that, due to a substantiated allegation of child abuse or neglect, you were disqualified from providing direct services to children or vulnerable-adult clients of DCS or DES.
